The Addison Police Department is committed to reducing traffic crashes and traffic violations by responding to citizens' traffic complaints. This form can be used to report speeding, red light, stop sign, reckless driving, and other ongoing traffic violations. If the problem you are reporting is in progress, please call the non-emergency number for the Addison Police Department at (630) 543-3080 to report it.

The following items marked with an asterisk (**) are required for the Addison Police Department to properly investigate your complaint.
